This notice is published pursuant to the Alberta Health Care Insurance Act.
As an opted-out physician, Dr. Ian Lund will not bill Alberta Health for insured medical services.
Patients seeking care from Dr. Ian Lund will be responsible for payment directly. Patients are encouraged to contact the clinic directly for information regarding fees and billing arrangements.
Dr. Ian Lund intends to practise in Alberta.
The required written notification to the Minister has been completed in accordance with section 3(a) of the Alberta Health Care Insurance Act.
This publication fulfils the requirement under section 3(b) of the Act.
